We at Diablo Valley Oncology / Hematology Medical Group believe that the stress of beginning cancer therapy can be reduced if the treatment space is patient centered, peaceful, intimate, and beautiful. Because we designed our center from the ground up, our infusion suite is lovely and calming, yet state of the art and extremely functional. All forms of medical therapy (chemotherapy, immunotherapy, hormonal therapy, experimental therapy and supportive therapy) are administered to patients in individual private spaces with plenty of room for family members. Reminiscent of a five star airport lounge, the setting offers comfortable reclining chair/beds, soothing music overhead, DVD players, and wireless internet access.
Because we are the largest single site of cancer treatment in the county with seven physicians all located at the same site, we have medical coverage at all times while patients are receiving treatment. In addition, five compassionate and technically-expert chemotherapy certified nurses are right there, constantly monitoring and assisting patients, which results in the highest nurse to patient ratio and the absolute best care. Meet the Staff
- Matthew N. Sirott, M.D.
- Dr. Sirott was born and raised outside of Philadelphia, Pennsylvania. He graduated from a six-year medical program, receiving his B.A. from Lehigh University and his M.D. from the Medical College of Pennsylvania. After completing a three-year residency in Internal Medicine at Montefiore Medical Center of the Albert Einstein College of Medicine in New York City, he completed his oncology fellowship at Memorial Sloan-Kettering Cancer Center and his hematology training at New York Hospital, Cornell University Medical Center.
- At John Muir Medical Center, Dr. Sirott is the Chairman of the Pharmacy & Therapeutics Committee. After serving for fourteen years, he recently resigned as the Chairman the Cancer Committee and Director of Oncology, to allow more time for development of the comprehensive services available at the California Cancer and Research Institute. Dr. Sirott is on the board of the Bay Area Cancer Research Group and is a principal investigator participating in research and clinical trials.
- Robert L. Robles, M.D.
- Dr. Robles was born in Philippi, West Virginia and raised in Tucson, Arizona. He graduated with a B.A. in Chemistry from Pomona College in Claremont, California and received his M.D. from Jefferson Medical College in Philadelphia, Pennsylvania. His internship and residency were completed at Milton S. Hershey Medical Center and Pennsylvania State University in Hershey, Pennsylvania. Following this he completed his fellowship in Medical Oncology / Hematology at the University of California at San Francisco where he also served as a Clinical Instructor. He has been a clinical assistant professor at both UCSF and San Francisco General Hospital. He is principal/secondary author of eleven original manuscripts and publications and two book chapters.
- Dr. Robles has been in private practice since 1997 and is a senior partner at Diablo Valley Oncology / Hematology Medical Group, located at the California Cancer and Research Institute in Pleasant Hill which specializes in comprehensive, cutting edge treatment of all forms of cancer and blood disorders.

- Dr. Catalya graduated with a B.S. in Pre-med from the University of San Carlos at Cebu City, Philippines and received her M.D. Degree from the Cebu Institute of Medicine. She completed her internship and residency in Internal Medicine at both Chong Hua Hospital in the Philippines and Jersey City Medical Center in New Jersey. Dr. Catalya also completed an Anatomical Pathology residency at Beth Israel Medical Center in New York. She then finished her Medical Oncology and Hematology fellowships at St. Michael's Medical Center in New Jersey.
- Dr. Catalya has been in private practice for over 20 years in the East Bay. - Sachin S. Kamath, M.D.
- Dr. Kamath grew up in Sarasota, Florida where he attended New College, the Honors College of the Florida State University system. During his time at New College, Dr. Kamath was awarded research fellowships at the Medical College of Georgia, and at the National Cancer Institute. He was then accepted into an accelerated medical program at the University of Florida in Gainesville. After graduating from medical school, Dr. Kamath completed an Internal Medicine internship followed by a Radiation Oncology residency at the University of Florida.
- Dr. Kamath has been in private practice since 1998. He joined Diablo Valley Oncology / Hematology Medical Group in 2008. Dr. Kamath is the Medical Director of Diablo Valley Oncology's state-of-the-art radiation oncology center which is located at the California Cancer and Research Institute in Pleasant Hill and specializes in comprehensive, cutting edge treatment of all forms of cancer and blood disorders. - Michelle received her B.A. in Psychology from Gonzaga University in Spokane, Washington in 1998 and her M.S.N. in the Family Nurse Practitioner program from Samuel Merritt College in Oakland, California in 2002. She is an Oncology Certified Nurse and has been practicing as a Nurse Practitioner since 2004 with Diablo Valley Oncology / Hematology Medical Group located at the California Cancer and Research Institute in Pleasant Hill which specializes in comprehensive, cutting-edge treatment of all forms of cancer and blood disorders. Michelle is a sub-investigator participating in research and clinical trials with Bay Area Cancer and Research Group at the California Cancer and Research Institute.
- Michelle serves on the Professional Advisory Board for the Wellness Community in Walnut Creek and serves on the Board of the local melanoma foundation, SCARF, as a medical advisor. Michelle takes great interest in the role exercise and nutrition play in the prevention and management of cancer and has published a review article on the subject in the Clinical Journal of Oncology Nursing.

- Gila received her Registered Nursing degree from University of California San Francisco in 2005 and earned her M.S.N. in the Gerontological Nurse Practitioner Program at UCSF in 2008. Medical Oncology

Chemotherapy
At Diablo Valley Oncology / Hematology Medical Group, we administer all forms of therapy in our beautiful new infusion suite. Our nurses are all certified and a medical practitioner is always on site when treatment is delivered. We administer standard agents, experimental agents, and supportive drugs, allowing almost all of our patients to remain out of the hospital. The newest antiemetics have relegated nausea to a problem from history. Supportive drugs reduce the incidence of low blood counts and fevers, and intravenous antibiotics can be delivered as an outpatient, if necessary. Although unwanted side effects are still present, their intensity is markedly diminished from ten or twenty years ago, which has allowed the widespread delivery of chemotherapy in many more sub-types of cancer. The physicians at Diablo Valley Oncology / Hematology Medical Group treat all types of cancers, and almost all of those treatments can be delivered within our premises.
Chemotherapy is the term used for drugs that kill cancer cells. In fact, these days, the term usually includes biologic or immunologic agents, also called targeted agents. In general, chemotherapy functions by interfering with a cells ability to reproduce, often by disrupting DNA replication. The side effects are noted because the drugs are active against both abnormal (malignant) cells and normal tissue. Targeted agents act on specific proteins which are on the malignant cells, but not on normal cells; often, this reduces the incidence of serious side effects.In many cancers, chemotherapy has markedly increased cure rates, length of life, and our palliative ability (relief or prevention of symptoms caused by cancer).
Blood Disorders
Hematology, the study and treatment of blood disorders, is usually divided into malignant and non-malignant diseases. At Diablo Valley Oncology / Hematology our physicians have expertise in treating both types of illness. Cancer medicine and blood medicine are usually treated by the same physicians because, back in the 1950's and 1960's, the first medically treatable cancer was leukemia, a blood disease cared for by hematologists (internists who treat blood disorders). Over the years oncology has developed into its own specialty, but hematologists remain at that forefront of administering treatment to patients with malignant blood disorders, as well as benign illnesses. We have board certified hematologists who enjoy seeing the full range of blood disorders.
Malignant blood diseases encompass a wide range of leukemias, both acute and chronic; lymphomas, ranging from low grade to aggressive; and a variety of other diseases, including myelodysplasia, Polycythemia Vera, Multiple Myeloma, and others. These illnesses require expertise both for diagnosis and treatment. While many other hematology/oncology specialists refer these patients to distant university settings, we at Diablo Valley Oncology / Hematology feel these patients are best treated locally. We are comfortable with all of these disease manifestations and are up-to-date on the most current therapies. We often enroll these patients in clinical trials, allowing them access to experimental therapies when needed. Our physicians have been treating these diseases for over twenty years. While at times, we do refer to the universities for very specialized treatments such as bone marrow transplants, we feel that most patients are better served when treated in their community, with family and our supportive staff immediately available.
Benign blood disorders usually manifest as abnormalities in the complete blood count (CBC). More specifically, patients usually develop a low hemoglobin (anemia), a low white blood count (neutropenia), or low platelets (thrombocytopenia). Other types of benign blood disorders involve abnormalities in the clotting system, which can result in too much clotting (a hypercoagualble state) or not enough clotting (a bleeding disorder). Hypercoaguable state research has been a rapidly progressing field, because of its impacts on obstetrics, in terms of unexplained miscarriages, as well as its impacts on stroke and heart attack patients. Hematologists often see patients with these types of abnormalities (which may appear to be unrelated to blood disorders), and are instrumental in the diagnosis and treatment of the underlying pathology. To assist in diagnosis, a procedure called a bone marrow aspirate and biopsy is sometimes necessary. We have performed hundreds of these procedures, and patients often comment that the procedure was surprisingly simple and straightforward, especially if they have had a previous experience elsewhere. Our nurse practitioners take extra time to provide the utmost in patient comfort during the procedure, which usually takes less than ten minutes. Radiation Oncology
Comprehensive treatment of cancer requires not only therapy aimed at the disease, but also careful attention to the individualized needs of the patients and their families or caregivers. Diablo Valley Oncology / Hematology Medical Group's Radiation Oncology program at the California Cancer and Research Institute brings together a unique group of warm-hearted, experienced medical professionals who are dedicated to providing compassionate, competent, and innovative clinical care by using proven, state of the art technology to deliver cutting-edge therapy.
In the treatment of cancer, Radiation Oncology uses sophisticated technology to aim high energy X-rays directly at the site of disease. Administering radiation therapy requires close-knit cooperation amongst an entire team of healthcare professionals, including radiation oncologist, nurse / medical assistant, physicist, dosimetrist, and radiation therapists. What distinguishes us from other radiation therapy programs is that our Radiation Oncology team works solely at the California Cancer and Research Institute and they along with the radiation oncologist provide full-time, on-site physics, dosimetry, and therapy support. As part of our mission to provide compassionate, first-class, fully integrated cancer care under one roof, we help coordinate multidisciplinary care plans with surgical and medical oncologists, other specialists, and the primary care physician.
Our cutting-edge treatment delivery is made possible through the use of two new Elekta Synergy linear accelerators which are equipped with cone-beam CT and respiratory gating technology.Our state of the art facility is also equipped with a CT/ PET simulator for sophisticated treatment planning. To demonstrate our commitment to technical excellence, we offer image-guided radiation therapy (IGRT), intensity modulated radiation therapy (IMRT), three-dimensional conformal radiation therapy, and stereotactic radiosurgery. These innovative technologies require our Radiation Oncology team to perform rigorous quality assurance checks to ensure that our patients get the maximal benefit. Treatment reproducibility, which means that each patient consistently receives the right amount of radiation to the intended target during the entire course of treatment, is critical. Second Medical Opinion
A second medical opinion is an important part of becoming educated about your cancer and your treatment options. The more you can learn about your diagnosis and your treatment options, the better chance you have of receiving the most appropriate treatment. Cancers are now more treatable than they once were, but there are also many more treatment options and more complicated procedures. Getting a second opinion will help you understand these options and help you make an informed decision about which is best for you. Second opinions will not offend competent physicians. Second opinions will, however, provide reassurance to you and your family and ultimately allow you to receive the most appropriate therapy.
What is a Second Opinion?
A second opinion is a review of the cancer diagnosis and the treatment recommendations of the physician who is treating the cancer by another, independent physician. Either the patient or the primary physician can initiate the process of getting a second opinion. Usually, patients obtain a second opinion after being referred to a second physician or to a special team of experts in a cancer center. This doctor or team of doctors will review the following:
* Pathology report (how the cancer looks under the microscope)
* The extent of cancer
* The physical condition of the patient
* The proposed treatment
The doctor(s) then communicate their opinion regarding treatment to both the patient and the primary physician.
Why Do I Need a Second Opinion?
A second opinion is part of the education process that is critical for cancer patients. The treatment of cancer has evolved tremendously in the recent past. As a result, many cancers are now more treatable than they once were, especially if the appropriate initial treatment is selected. In order to receive appropriate treatment, patients must understand the type of cancer they have and the treatment options that are available. However, there are also many more options for treatment and these options are more complicated than in the past. For these reasons and others, it is advantageous to seek more than one opinion about how your cancer can be treated. Also, a second opinion provides the opportunity to get information from someone other than the physician who will be directing treatment, which is usually the main source of information for most patients. Second opinions are a common practice in any area of medicine that is complex and that has multiple treatment options available.
PET / CT & Diagnostic Imaging
Diablo Valley Oncology Medical Group's patients are benefitted by on-site access to a state-of-the-art “high definition” PET-CT imaging system. PET-CT is, for most cancers, the best imaging modality available for diagnosis and staging at the time that a patient's tumor is first discovered. Further, it is often the preferred technique to evaluate the cancer's response to treatment. The Diablo Valley Oncology PET-CT system, the Biograph TruePoint 6 manufactured by Siemens, has features that place this system at the top of the Bay Area's offerings in terms of sensitive and accurate cancer imaging. Several of the technologies are unique among Bay Area systems, and are not even available at the university medical centers.
Our high definition PET yields outstanding detail and clarity, with less unwanted noise and distortion, in comparison to the usual scanners more readily available in Northern California. Additionally, because more of the body is covered in each step of the imaging procedure, the scan times are shorter with less radioactive isotope administered per patient. The CT component of Diablo Valley Oncology's PET-CT system affords state-of-the-art multi-detector computed slice imaging, maximizing the performance and minimizing radiation exposure when operating in CT-only mode.
The actual technique of PET (positron emission tomography) scanning is unique in medicine. A sugar molecule is bonded to a radioactive element (which releases positrons). After the sugar is injected, it is taken up by the active cells within the body. The reason the scan is so successful is that cancer is, in general, more metabolically active than other body tissues, and so takes up more sugar. As an aside the heart, brain and muscle light up on PET scans because they too are very metabolically active. After the cancer takes up the sugar, the radioactive element is retained in the tissue and then positrons are released and visualized by the scanner. A breakthrough in radiology was developed in 2003, when PET scanning was performed on a combined PET and CAT scan machine, enabling more precise localization of the abnormal uptake. CT scanning (computed axial tomography) is strictly an anatomic imaging; it can only reveal the presence or absence of tissue, but cannot determine whether this tissue is malignant. The combination of a metabolic scan (the PET scan) and an anatomic scan (the CAT scan) assists in the determination of where any abnormal tissue is located and whether or not that tissue is malignant (cancerous).
An extremely important component of the radiographic evaluation of a cancer patient involves the skill of the radiologist, the person who interprets the scan. Dr Barry Engelstad has been the director of the center since its inception in 2003. As a prior associate professor of radiology and nuclear medicine at the University of California, San Francisco Medical Center for 9 years, he was already an expert in his field. Because our scanner is one of the busiest in the country, Dr. Engelstad has now had more experience than almost any other physician at reading these combined scans. His knowledge and experience, combined with the excellent technology, yields the highest quality radiology results attainable anywhere. Furthermore, our physicians, technicians and staff have been working together since the inception of the center. Steve and Nick care about each and every patient and strive to make your visit as pleasant an experience as possible, while ensuring exacting and meticulous adherence to protocol. The executive director, Lora Sanders, ensures that the center operates smoothly; she is always available to try to assist with problems and improve the center in any way possible. Even if you do not receive your cancer treatment at the California Cancer and Research Institute, you should certainly obtain your imaging here, so as to avail yourself to Dr. Engelstad and his team's expertise.
Clinical Trials / Research
Clinical Research is an integral part of the cutting edge medical therapies delivered by the physicians at Diablo Valley Oncology / Hematology Medical Group. Most clinical trials utilize new drugs, especially the most recent agents, the so-called targeted therapies, to try to improve outcomes in cancer patients. Some trials look at new radiation therapy applications or novel combinations of radiation and medications. At Diablo Valley Oncology / Hematology Medical Group, we are committed to obtaining access to these new treatments for the benefit of our patients. Patients are offered these trials as an option when available, and we are proud of our high quality studies. Because our research corporation, Bay Area Cancer Research Group, is well organized, nimble, and delivers high quality data, the pharmaceutical companies sometimes prefer utilizing our site over universities, such as UCSF and Stanford. In fact, we sometimes get referrals from them to be enrolled in our studies!!

Balanced nutrition is a very important part of cancer treatment, as well as survivorship and prevention. A healthful diet can help rebuild your body's cells and energy level, especially if you are receiving chemotherapy or radiation treatment. We encourage you to talk to our nutrition expert while receiving your care at Diablo Valley Oncology / Hematology Medical Group. Our registered nutritionist has special training in the area of oncology nutrition and working in association with your physician will design a practical eating and supplement plan to meet your individual needs with special attention to your beliefs and cultural values.
Exercise is no longer complementary, but an integral part of therapy. Large clinical trials with hard clinical data have shown a benefit in at least two cancers in regards to a reduction in cancer recurrence rates in patients who already carry a diagnosis of cancer. Additionally, exercise during therapy has improved patients ability to complete therapy and to decrease side effects. We at Diablo Valley Oncology / Hematology Medical Group believe this is so important that we are in the process of formalizing these recommendations. We recognize that living with cancer is a challenging experience which is why we often suggest supportive therapies such as yoga, massage, meditation, acupuncture, psychotherapy, and counseling. These approaches are finding an effective place in cancer treatment – not necessarily to increase cure rates, but rather as complementary therapies that may help improve patients overall well-being and tolerance of therapy. For example, some patients have found that acupuncture has helped manager chemotherapy associated nausea and vomiting and helped to control pain associated with surgery. Meditation may improve sleep, resulting in higher quality of life. We encourage you to share your individual challenges with your Diablo Valley Oncology / Hematology Medical Group physician and nurse practitioner so that they can recommend complementary therapies that may be of benefit to you.
